Health Justice Network, New York City Department of Health and Mental Hygiene
The New York City Health Justice Network (HJN), a program to improve the health and well-being of persons involved with the criminal legal system by increasing their access to, engagement in, and the quality of community-based health and social services to support the re-entry process, engaged JLA to provide guidance and technical assistance on designing, establishing and implementing a community of practice for the HJN partner agencies. JLA is also serving as a technical advisor on HJN measurement and improvement strategies, and delivering training on improvement and evaluation methods.
Coordinated Behavioral Care
Coordinated Behavioral Care (CBC), a large behavioral health Independent Practice Association, engaged JLA to develop a Quality and Performance Management Roadmap for achieving QPM goals delineated in CBC’s recent strategic plan. JLA interviewed staff, reviewed CBC materials, and conducted a scan of the grey and peer-reviewed literature to promulgate a set of Roadmap recommendations addressing quality improvement approaches, performance measurement and network provider engagement. JLA also provided consultation to CBC on the implementation of select recommendations.
Planned Parenthood Federation of America
The Clinical Quality Improvement (CQI) department at Planned Parenthood Federation of America (PPFA) to facilitate a strategic planning process. JLA activities included working with the CQI team to clarify current state and strategic planning objectives, examining CQI documents capturing work to date, conducting interviews with select PPFA affiliates, and designing and convening a strategic planning event for key staff.
Health Leads engaged JLA to advise on their collaborative learning strategy for spreading social needs integration in clinical settings. JLA shared best practices and alternative approaches for consideration, conducted staff interviews to understand current practices, and formulated recommendations for refining their collaborative learning strategy. JLA facilitated a half-day workshop with leadership to delve into the recommendations and incorporated feedback into a final set of recommendations. JLA also provided improvement advising for Health Lead’s Boston Foundation (TBF) Collaborative.
